The University of Lahore uses its own grading table, not the HEC one. A is 85 and above (4.00), A- is 80 to 84 (3.75), B+ is 75 to 79 (3.50), and the grades then step down in blocks of 5 marks to D at 50 to 54.

UOL has no B- or C- grades, and each band is 5 marks wide. Bands are half-open: 80 up to (but not including) 85 is A-. So 84.9 is still A-.
How UOL differs from the HEC scale
Several calculators online show the wrong table for UOL, either the HEC scale or a flat 80 = A. The official UOL table gives more points in the middle bands:

GPA = 45.75 ÷ 15 = 3.05.
One mark separates courses 1 and 2, but it adds 0.75 quality points. Course 3 at 69 is a C+; one more mark would have made it B and added 1.50 quality points.
Repeats and minimum CGPA
