eGFR (estimated glomerular filtration rate) shows how well your kidneys filter blood, in mL per minute per 1.73 m² of body surface. An eGFR of 90 or more is normal, 60 to 89 is mildly decreased, and below 60 for more than 3 months means chronic kidney disease. A 55-year-old woman with a creatinine of 1.2 mg/dL has an eGFR of about 53, stage G3a.

The CKD-EPI 2021 equation
The equation was published by Inker and colleagues in The New England Journal of Medicine in 2021. It replaced the 2009 version and removed the adjustment for race. The US National Kidney Foundation and the American Society of Nephrology recommend it.

Rounded to a whole number that is 53, stage G3a. A man of 50 with a creatinine of 115 µmol/L has 115 ÷ 88.4 = 1.30 mg/dL, and an eGFR of 67, stage G2.

Stages G1 and G2 count as chronic kidney disease only when there is also a sign of kidney damage, such as albumin in the urine, blood in the urine or an abnormal scan. Any stage needs to last more than 3 months to count as chronic.

The same creatinine gives a lower eGFR as you get older, because the equation expects kidney function to fall slowly with age.
eGFR in Pakistani patients
The CKD-EPI equations were built mostly from people in North America and Europe. Pakistani researchers have tested them against measured kidney function. A 2024 study in PLOS One of 385 adults with chronic kidney disease in Pakistan found that the 2021 equation was less accurate there than a version adjusted with local data, often called the Pakistani CKD-EPI. That adjusted version has not been adopted in international guidelines, so this calculator uses the standard 2021 equation.
Your lab may report eGFR with a different equation, such as MDRD or CKD-EPI 2009, so its number can differ from this one by a few points. Your doctor will go by trends over time, not a single result.
When eGFR can mislead
Creatinine comes from muscle, so eGFR can be wrong when muscle mass is unusual: bodybuilders, amputees, people who are very thin or bedbound, and people on a vegetarian diet. It is not valid in pregnancy, during a sudden illness that affects the kidneys (acute kidney injury), or in children, who need a paediatric equation. Eating a large meat meal or heavy exercise in the day before a test can raise creatinine for a short time.
If your eGFR is low, a doctor may repeat the test, check your urine for albumin, or order cystatin C, a second blood marker that does not depend on muscle.
This calculator does not diagnose kidney disease. Only a doctor can do that, using repeated tests over at least 3 months. See a doctor promptly if your eGFR is below 60, if it has fallen quickly, or if you have diabetes or high blood pressure, which are common causes of kidney disease in Pakistan and elsewhere.

What is a normal eGFR?
An eGFR of 90 or more is normal. Kidney function falls slowly with age, so an eGFR in the 70s or 80s is common in older adults and is not kidney disease on its own. An eGFR below 60 that lasts more than 3 months is chronic kidney disease, whatever your age.
How do I calculate GFR from creatinine?
Use the CKD-EPI 2021 equation: 142 × min(Scr/κ, 1)^α × max(Scr/κ, 1)^−1.2 × 0.9938^age, times 1.012 for women. κ and α depend on sex. For a 60-year-old man with a creatinine of 1.2 mg/dL, the result is 69. The calculator shows every step.
What stage of kidney disease is an eGFR of 50?
An eGFR of 50 is stage G3a, which covers 45 to 59 and means mildly to moderately decreased function. It counts as chronic kidney disease if it stays below 60 for more than 3 months. Many people at this stage have no symptoms, so regular checks with a doctor matter.
How do I convert creatinine from µmol/L to mg/dL?
Divide µmol/L by 88.4 to get mg/dL, or multiply mg/dL by 88.4 to get µmol/L. For example, 106 µmol/L is 1.2 mg/dL. The calculator converts for you when you pick the unit shown on your report.
Is eGFR the same as creatinine clearance?
No. Creatinine clearance is often estimated with the Cockcroft-Gault formula, which uses weight and is not adjusted for body surface area. It is still used for some medicine doses. eGFR from CKD-EPI is the measure used to stage kidney disease. Ask your doctor or pharmacist which one applies to your medicines.
